Personal vehicle is a requirement as travel to multiple program offices is required (mileage reimbursement included).
Position Description
Provides both indirect and direct clinical support to people with disabilities throughout Achieva Support. Assesses and reports clinical needs as needed. Educates’ staff and people supported on medical care requirements and health and wellness improvements. Acts as a liaison to internal and external healthcare professionals to promote health and safety to people supported. Participates in an on-call rotation to provide nursing mentorship and support outside of the agency’s regular operation hours.
Job Responsibilities – Highlights
Supports people served by Achieva’s programs, in achieving their best possible overall health and personal outcomes, using a person-centered approach.
Provides ongoing direct and/or indirect clinical support to people with disabilities throughout Achieva Support who are experiencing a range of clinical needs due to health diagnoses/symptoms, physical or mental decline, aging, and/or other medical or behavioral health needs.
Serves as a liaison between staff and external healthcare professionals and/or hospital and skilled care facilities when vital. Efficiently communicates with external professionals re: a person’s current medical diagnosis/symptoms; relays any medical needs identified, and provides any background medical history.
Conducts a detailed assessment of persons presenting with symptoms of an acute illness and/or psychiatric episode. Contact physicians when vital. Records and reports findings & recommendations to appropriate supervisor/manager of program, as well as to the clinical team. Provides follow-up monitoring to ensure resolution of identified issues/concerns.
Maintains comprehensive and accurate documentation of all medical notes to ensure the clinical team is apprised of identified situations/concerns, and all follow-up requirements and assessments as needed.
Participates in an after hours on-call rotation for Achieva Support programs as needed, along with clinical team RNs. Will provide mentorship & instruction to staff regarding episodic illnesses, emergency situations; accidental injuries sustained; and medication errors and/or questions.
Attends medical and/or psychiatric appointments for people supported, when requested. Advises applicable staff of the outcome of the appointment, then supervises to ensure all follow-up care/treatment is rendered as ordered.
Adheres to policies and procedures established by Achieva and Achieva Support, as well as government regulations, funder standards and licensure boards and acquires certification and proficiency in specialty roles when requested to do so by the Clinical Services Director.

Qualifications
High school diploma or equivalent plus Licensed Practical Nurse certification by the State Board of Nursing Examiners. Two years nursing experience preferred. Satisfactory Act 33/34 and FBI clearances, are required. Valid license to drive in the state of Pennsylvania and properly insured vehicle for daily use.
