● Rotational on-call weeks required

$60,000 yearly
Location:
Achieva Main Office(Southside)/Greater Pittsburgh area travel
Travel:
A personal vehicle is a requirement as travel around the Greater Pittsburgh area is required (mileage reimbursement included).
Position Description:
The Registered Nurse in community health is responsible for providing a variety of healthcare services and promoting wellness in the community. This role involves conducting health assessments, developing and implementing health education programs, providing clinical support, and collaborating with community-based health and educational organizations; i.e., healthcare facilities, teaching hospitals, disability centers of excellence, etc. to address any identified health disparities.
Key Responsibilities:
1. Health Assessments & Care Planning:
○ Conduct health assessments and screenings for individuals receiving supports from Achieva.
○ Identify health needs and risks, and develop/update care plans that reflect community health priorities.
○ Monitor critical and chronic diseases, including those such as dysphagia, dehydration, diabetes, hypertension, and asthma, etc., and help manage conditions with appropriate clinical interventions and staff education.
2. Health Education & Promotion:
○ Educate Achieva staff, families and the people receiving supports on health topics, such as nutrition, exercise, sexual health, immunizations, and disease prevention.
○ Organize and deliver health education workshops, seminars, and outreach programs.
○ Advocate for healthy lifestyle choices and preventive health behaviors.
3. Clinical Care:
○ Provide oversight and conduct routine monitoring and assessment of the person’s health and safety.
○ Provide needed health care and safety related training to the staff who render services.
○ Oversee access to and coordination of medical care including assisting the staff who render services, and be available for consultation on an as needed basis for emergencies.
○ No direct care/shift responsibilities unless required in an emergency situation.
4. Collaboration & Advocacy:
○ Work with other healthcare providers, social workers, and community organizations to improve overall community health for those supported by Achieva.
○ Advocate for policies and resources that benefit community health, especially for underserved populations.
○ Support community health initiatives and serve as a liaison between healthcare institutions and Achieva staff; persons receiving services and their families.
5. Documentation & Reporting:
○ Maintain accurate health records and documentation for those receiving support.
○ Complete health assessments, identify health trends, and provide evaluations.

Qualifications:
Education:
○ Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted.
○ Active Pennsylvania state licensure as a Registered Nurse (RN).
○ Additional certifications in public health nursing or community health nursing are a plus.
Experience:
○ Previous experience in community health or public health nursing is preferred but not required.
○ Strong clinical skills and the ability to work independently.
○ Experience in health education, program planning, or community outreach is an asset.
Skills:
○ Strong communication and interpersonal skills.
○ Knowledge of health issues affecting diverse populations and the ability to address cultural and socioeconomic factors in healthcare.
○ Ability to work collaboratively with individuals, families, and healthcare teams.
○ Competence in basic healthcare technology and electronic health records.
Work Environment:
● This role may involve traveling to community settings such as schools, clinics, health fairs, and community homes.
● Occasional evening or weekend hours may be needed for community outreach programs; individual health emergency, and/or pandemic preparedness and response.
