Function:
Responsible for conducting agency investigations as outlined by all laws, regulations and agency practices. Participate in investigative administrative reviews and quality assurance peer reviews to ensure incident management processes meet regulatory standards. Provide hands-on assistance and guidance to the agency’s adjunct certified investigators. Provide assistance as needed to Achieva Support’s Incident Manager and Incident Management Representative. Ensure knowledge of and compliance with the Office of Developmental Programs (ODP), incident management requirements, and protective services regulations and processes. Reports to the agency’s Incident Manager.
Responsibilities:
Conducts certified investigations for agency incidents as outlined by all laws, regulations and agency practices; including reporting of incidents to protective services & local law enforcement as required.
Complies with required incident management reporting timeframes; including ensuring incidents are filed, finalized and/or extended according to the timelines set forth by laws, regulations and/or standards.
Tracks timeframes and timeliness of incident Enterprise Incident Management reports/submissions. Reports all incident negative trends and implements corrective action plans accordingly.
Prepares and coordinates state incident peer review audits as required.
In conjunction with the Incident Manager, assists in the oversight of agency adjunct Certified Investigators.
Orients new agency adjunct investigators to their role and provides on-going coaching throughout the investigation process.
Coordinates quarterly Certified Investigator’s meetings. Conducts meetings as directed.
Tracks and ensures required state certifications are maintained by all Certified Investigators.
Documents and prepares meeting minutes for all Incident Management/Certified Investigator related meetings; peer review findings; etc.
Collects data as needed for monthly, quarterly and end of year Incident Management reviews & trend analysis.
Ensures all documentation is present and properly stored and labeled.
Ensures chain of custody is maintained of all investigation materials, and that confidentiality is maintained.
Provides after hours on-call support as needed regarding the Incident Management process and reporting of incidents.
Determines if an incident meets the criteria for reporting; and if an investigation is required to be filed and initiated for an incident.
Maintain current Pennsylvania Certified Investigator qualification in accordance with Office of Developmental Programs Incident Management Bulletin requirements.
Performs other duties as assigned.
Qualifications:
Associate degree in human services, social science, and/or related fields required. Previous work experience in protective services, aging, autism and/or intellectual developmental disability industry preferred. Must be able to work flexible hours including weekend and holiday hours when an investigation need arises. Satisfactory Act 33/34 and FBI clearances are required. Valid driver’s license and properly insured vehicle for daily travel when completing investigations.
